CHICAGO — The Illinois Department of Financial and Professional Regulation has issued its discipline report for June.
Cottonwood Financial Illinois, LLC, in Mt. Vernon was fined $4,000 for the following violations: Lender did not input accurate information into the database to determine if the obligor was eligible for a loan; the lender did not obtain any official documentation of the borrower’s income before making a payday or title-secured loan; lender made an installment payday loan exceeding 22.5 percent of the obligor’s gross monthly income or lender made a payday loan exceeding 25 percent of the obligor’s gross monthly income.
In medical displines, the physician and medical license of Darcey Kobs of Mt. Vernon was placed in refuse to renew status after being disciplined by the state of Texas following DEA findings of illegal operation of his clinic as it related to prescribing of controlled substances; and the physician and surgeon license of Robert Sexe of Mt. Vernon was placed in refuse to renew status after being disciplined by the state of Missouri.
And, the registered nursing license of Barbara Jones of Mt. Vernon was restored to indefinite probation for a minimum of three years.
