Bethel School registration set

All students attending Bethel Grade School District 82 during the 2014-15 school year will register on Monday, 21 from 10 a.m. to 6 p.m. Parents should enter the east doors of the building and proceed to the cafeteria to register.

The registration fee of $25 is to be paid at the time of registration.

All parents/guardians will complete an assurance of residency at the time of registration and provide a proof of residency document. Proof of residency includes but is not limited to a residency questionnaire and affidavit that is notarized, lease agreement, mortgage papers, utility bills in your name and a property tax bill for residence or mobile home. All new students to Bethel Grade School must present a copy of their birth certificate, plus provide three documents as proof of residency. Any returning Bethel students who have changed their location of residency, but still reside within the District, must also provide three documents for proof of residency.

Parents of kindergarten students need to register their child. Students entering kindergarten must be 5-years-old on or before Sept. 1, 2014. All kindergarten students must have a copy of their birth certificate, physical, immunization record and dental records on registration day. A lead screening is also required for kindergarten students. Also, kindergarten students are required to have a vision exam performed by a licensed eye doctor.

Teacher workshop day is Thursday, Aug. 14. The first day of school is Friday, Aug. 15, from 8:20 a.m. to 3 p.m. Students should have registered on July 21 and be prepared to attend school on Friday, Aug. 15. School will dismiss at 3 p.m. on Friday, Aug. 15.
